Terms and Conditions
I acknowledge and agree to the following conditions.
All children must be formally enrolled through Aimyplus
All children are to be collected no later than 5.30pm each day. Or if booked in till 3.00 pm at the Holiday Programme no later than 3.10pm. A penalty fee may be charged for failure to collect children by this time as per the Payment of Fees Policy.
Fees must be kept up to date or paid in advance. Failure to do this can result in the child/ren being stood down until full payment is made.
Only persons appropriately identified as authorised people will be allowed to pick up a child unless the Programme Manager has been previously advised, preferably in person.
The Manager must be notified if a child will not be attending on a booked day before 8.30 am. (Leave message on the answer phone). There is no absence fee and you will be charged the normal daily fee. If you have booked your child in and then you need to cancel a one week notice will be given (unless otherwise organised with the Manager) to enable a refund to be made.
When applying for WINZ subsidies it is the parent’s responsibility to pay the Programme fees until authorisation from WINZ has been received. This will then be refunded from the date that WINZ begins payment.
Any disagreement regarding fees is to be addressed to the Programme Manager.
The Programme Manager must be advised immediately of any change in details contained in the Enrolment form.
The Programme Manager will be advised of any situation that may be disturbing to the children.
It is ultimately my responsibility to ensure that my child/ren act in an appropriate manner while at Kids Alive. Behaviour which consistently affects the quality of care to other children may result in suspension or removal from the programme as set out in the Behaviour Management Policy.
The Programme Manager may arrange for urgent medical treatment at my expense.
I agree to pay for damage wilfully caused by my child.
I understand my child is responsible for his/her own property.
I give permission to go to prearranged outings. This may include trips to Caroline Bay, CBay and other public places. These will be either walking or transported by bus/mini bus. I understand that at all times the staff/child ratio will be adhered to.
I give permission to go on equipment such as the obstacle course bouncy castle and climbing wall.
Photographs of my children may be used for genuine resources and publicity purpose unless I have advised otherwise on my child’s profile through Aimyplus.
I give permission for my child to watch the occasional video, DVD, rated G or PG only.
I agree to abide by the rules and procedures of Geraldine Family & Youth Trust Kids Alive
(a copy of all current policies and procedures can be obtained from the Manager).
I understand that all staff are screened, trained and are gaining appropriate experience for the job they do and ‘Holidays @ Kids Alive’ complies with Health and Safety Policies and the National Oscar Standards. However accidents do happen and I will not hold the Geraldine Family & Youth Trust, the operators of ‘Holidays @ Kids Alive’ responsible for genuine mishaps.
I understand that the Kids Alive programme is managed by the Geraldine Family & Youth Trust which is completely separate from the Geraldine Primary School. Neither the staff nor the Board of Trustees of Geraldine Primary School are responsible for the Kids Alive service, nor liable for any act or omission arising through its activities.
